Shared Health has developed Community ConnectionTM, a community health record (CHR) that allows different providers treating the same patient to view medical, lab and prescription claims information and immunization records from multiple sources via a secure Web site. It connects medical professionals to a database that includes patient health care histories beyond what their own practices can provide. The service will be available at no cost to patients or providers.
With Community Connection, providers can determine what tests, treatments and diagnosis a patient may have received from other providers. Shared Health removes a key obstacle in the health care delivery system – a lack of information that impedes health care decisions, drives up costs and increases the likelihood of errors.
Shared Health meets all HIPAA and other privacy regulations, and all information is transmitted securely. Providers can access the data from most PCs with a high speed Internet connection using the proper security login and encryption codes.
